They explain in the video, but they're aestheticians not dermatologists. So they're "legally" not allowed to open it too much. Though I've seen other aestheticians open more.

This was indeed a very satisfying video. I love the way the gooey brown gunk piles up on the longer squeezes. Perhaps a bigger hole? Don’t really need to worry about scarring on the back. Still one of the best. 9/10.
