He has a condition called steatocystoma multiplex. Which is the formation of epithelial-lined, sebum filled, dermal nodules. This is a chronic condition and he will need extractions for the rest of his life to keep them controlled. Most of these have very thin sacs (some you can see pop out when I squeeze). This procedure is relatively painless. Several factors to consider. Injecting lidocaine to over 50 locations can be very painful. If you remove the sac there is a risk of indentations that some patients hate worse then the bumps. This patient has a history of keloids on the chest from previous removals elsewhere. Getting the sac out means a bigger cut at the surface. Due to these factors I chose not to remove his sacs after discussing the risks with him due to his risk for keloids. He was happy with the choice because he just wanted the bumps flattened and he realizes this is a chronic condition we will have to revisit. Thank again to my patient for sharing. Steatocystomas.
