First...gloves should have been worn to protect the patient. Second, this did not look like a boil at all. I had a boil once. When it got squeezed there was a hard core that came out. The same for my brother. He had a small solid core come out. This was not a boil.
That looks like it was messed with repeatedly for quite some time. This guy was lucky to have removed the sac. Hopefully this healed up and his leg went back to normal.
